1.                   Read through all the following agreements:

·         Risk Disclosure Statement for Futures and Options.

·         Customer Agreement.

·         Privacy Policy Notice.

o        If you are establishing a consumer account for personal, family, or household purposes.

·         Electronic Trading and Order Routing Systems Disclosure Statement.

·         Arbitration Agreement.

o        Note that the Arbitration Agreement does not need to be signed in order to open an account. Remember to check the box at the bottom of the Arbitration Agreement if you elect to resolve disputes through arbitration.

2.         Complete the Individual Customer Account Agreement. Required fields indicated by red askerisk.

3.         Complete the Account Authorization and Treatment of Funds.

4.         Complete the Hedge Customers Hedge Confirmation Letter.

5.         Complete the IRS Form W-9.

6.         Complete the  Authorization for Delivery of Statements by Electronic Media if you wish to receive your statements by e-mail or fax.

7.         Complete the Automated Clearing House (“ACH”) and Wire Transfer Instructions.

8.         Complete the Signature Page by doing the following:

a.                   This electronic account form allows you the ability to digitally sign this agreement by checking the ‘I acknowledge’ check box. Please note that a new account will not be established unless you have digitally signed this agreement.

b.                   Remember to print a copy of this agreement for your records.

c.                   A PDF copy of this agreement will be e-mailed to you upon submission of  this agreement for approval.
